You have been  selected to join Healthier You: the NHS National Diabetes Prevention Programme.

Type 2 diabetes can cause long-term health issues but is often preventable with simple diet and lifestyle changes. The Healthier You programme helps you make these changes, along with thousands who joined in the last year.

you can register and start your journey immediately to reducing your risk of developing type 2 diabetes.

You do not need to contact the practice to self-refer to the NHS Diabetes Prevention Programme.

Healthier You: NHS Diabetes Prevention Programme

Healthier You is a free NHS programme offering personalized support to help prevent type 2 diabetes. Backed by global research, it has already helped over 200,000 people. Of those who start, 70% lower their blood glucose to a healthy range, reducing their risk of diabetes.

The programme is delivered in two ways:

  1. Group-based Programme: Connect with others on your journey to better health.
  2. One-to-one Digital Programme: Receive personalized support from an expert health coach via phone, video calls, or in-app messaging.
